

Patients may be referred to BridgePoint Health by calling either our Sheboygan or Chilton offices. Patients may also call to arrange an appointment on their own. Inpatient consultations may be scheduled for patients at St. Nicholas Hospital.
The initial evaluation and assessment will consist of about a 45 to 50 minute clinical interview with a psychiatrist, a psychologist, or a therapist. In addition, assessment questionnaires may be given to a patient (depending on his or her presenting complaint) to complete either at the clinic (in a designated area) or at home. Occasionally, additional cognitive or personality assessment may be indicated. This will be performed on an as needed, case-by-case basis and will be discussed with the patient. The initial evaluation is designed to determine the appropriate level of behavioral health care and the selection of appropriate treatment procedures. Accordingly, if a patient is initially seen by a psychologist or a therapist, it may be necessary to have a psychiatrist conduct an additional evaluation to determine the need for medication or other necessary medically-related tests or interventions. Similarly, if a patient is initially evaluated by a psychiatrist, a referral to a psychologist or therapist for treatment may be indicated, either as the exclusive form of treatment or as an addition to medication.
Treatment
Treatment is intended to be time-limited with an emphasis on the development of individual self-coping skills and the reduction of symptomatic distress. The general goals of treatment are to help patients manage symptoms, become more independent and self-reliant, and enhance self-control as quickly as possible. Patients are matched with clinical staff that has expertise or specialized training in the treatment of their specific problem or concern. The BridgePoint Health staff will consult with the patient’s physician to facilitate close cooperative work. Any necessary medical follow-up will be arranged in a timely and appropriate fashion in consultation with the patient’s referring physician. Likewise, any additional need for group, family, or psychiatric evaluation will be determined on a case-by-case basis. Treatment of alcohol or other substance abuse problems related to anxiety or a medical condition will be provided by appropriate support staff. Treatment involving medication will be initiated, as indicated and appropriate, by a psychiatrist.
Follow-Up
Communication will occur with the patient’s referring physician regarding treatment plans and recommendations, as well as final treatment summaries and discharge planning.
